June 16, 2026Catholic Health Australia members were among 30 providers to present case studies on how they deliver quality palliative and end-of-life care.
The ELDAC Linkages Showcase Program was an opportunity for aged care members to outline the success of their work to each other, as well as to providers outside the not-for-profit and charitable sector.
The event was organised by ELDAC which is a sector-wide program that provides information, guidance, and resources for all aged care staff to support palliative and advanced care.
“CHA members are among many of our great services sharing examples of how they have included ELDAC resources into their systems to assist staff in end-of-life conversations and clinical care’, said Senior Advisor Aged Care, Shona McQueen.
Southern Cross Care NSW/ACT St Joseph’s Residential Aged Care provided an update on how it upskills staff to deliver quality palliative care, and ensure families are engaged during the end-of-life pathway for its residents.
“A take home message from Southern Cross Care NSW/ACT St Joseph’s Residential Care was ‘Living Well Leaving Well’. All participants thought this was a great line that will likely be used across many services”.
Keon Park Mercy Place also highlighted how the ELDAC Linkages Program helped increase knowledge for staff and clinicians to assess frailty and how multiple co- morbidities impact palliative care needs.
