November 21, 2024In times of rapid change, having quick and reliable access to the right information and resources on end of life care is more important than ever.
With upcoming updates to the Aged Care Quality Standards, the introduction of a new Aged Care Act, and changes to the Home Support Program, there’s a growing need for streamlined, accessible tools.
The national specialist palliative care and advance care planning advisory service, ELDAC, is updating it’s resources so:
- Care staff can easily find practical resources to support their work with older Australians.
- Service providers are equipped to meet new standards and build strong, capable teams.
- The aged care sector has the tools needed to navigate reforms and address ongoing challenges.
As part of its Knowledge Hub Project, more resources will be made available over the coming months.

ELDAC Linkages Program
The ELDAC Linkages Program is a facilitated palliative care and advance care planning initiative that aims to improve the quality of end of life care for all older Australians receiving aged care.
New ELDAC Linkages videos provide practical tools and inspiring stories to enhance care for older people. Here, carers will be able to explore key questions from the videos to drive meaningful change.
Catholic homes
In July, staff working in Catholic aged care homes across Australia were among more than 150 people to attend the annual ELDAC Linkages Project Showcase in Canberra.
